I do not know if this is coincidental. I had developed a very similar Service Request form 2 years ago, along with Activity Based Costing Model, where we have now packaged our EBMS [Environment Build and Management Services] offerings in the form of a tool for a customer of ours.

This form actually validates what my own development was. Thanks a ton!

We did a very similar offering for our customer too (actually all of our customers). But it was hidden. By which I mean they could select one of three tiers of services and we build the system like what is here. BUt we didn't have so much detail. And we didn't call it A, B, C. It was Diamond, Platnium and Gold. Amd our targets were not the same.
